If a delivered makhana pouch appears damaged, record the outer parcel and the individual pouch separately. Note what you can see, copy the batch and date markings exactly, and connect your photos to the seller and order reference. This creates a clear arrival record; a visual checklist cannot certify that food is safe.

How to use the worksheet

Copy the table into your own notes or onto paper. Make one record per affected pouch, numbering them if several arrived together. This page has no upload or submission form.

Describe the pack as you found it, before changing its condition where practical. Do not pull a seal apart to investigate a gap. If you have already opened the pouch, say so and distinguish what you noticed before and afterwards. Write “not visible” or “unreadable” when appropriate, rather than filling a gap from memory.

Describe the observation, not an assumed cause

“A split is visible along the pouch's left seam” is more precise than “the courier spoiled it”. An outer-carton dent and a pouch-seal gap are separate observations. Photographs can document what you saw, but they do not establish when damage occurred or who caused it.

A message you can adapt

This is an optional writing template, not a required complaint format. Send it through the seller's official contact channel. Keep address labels and order details private rather than posting them publicly.
